Approved uses in Japan and the distinction from off-label use
In Japan, approved indications include obesity under specified conditions and metabolic dysfunction-associated steatohepatitis with moderate or advanced fibrosis without cirrhosis. Obesity eligibility includes BMI and conditions such as hypertension, dyslipidemia or type 2 diabetes. The liver indication added in June 2026 must not be extended to everyone with fatty liver.
Official prescribing information/product documentation (4. Indications, 5. Precautions concerning indications).
Check the formulation and directions for use
The prescribing information directs starting once-weekly injections at a low dose and usually increasing stepwise every 4 weeks. Adjustments depend on tolerability; do not shorten the escalation interval to lose weight faster. Do not confuse operation of the SD pen with that of the multidose MD pen.
Official prescribing information/product documentation (6. Dosage and administration, 7. Precautions concerning dosage and administration).
When treatment may not work well and its limitations
Diagnosis and assessment of eligibility for obesity or liver disease are necessary. The prescribing information also notes that improvement in liver-related outcomes has not been verified; prevention of cirrhosis or future disease cannot be guaranteed.
Official prescribing information/product documentation (5. Precautions concerning indications, 8. Important precautions).
Reviewing the effect and whether to continue treatment
Monitor weight, associated health problems, nutritional status and side effects. For MASH, a specialist assesses fibrosis and other factors; weight alone does not establish successful liver disease treatment.
Official prescribing information/product documentation (8. Important precautions).
Side effects and symptoms requiring medical attention
Besides gastrointestinal symptoms, watch for acute pancreatitis, cholecystitis and other conditions, hypoglycemia and ileus. Seek advice promptly for persistent severe abdominal pain, repeated vomiting, severe constipation or abdominal distension.
Official prescribing information/product documentation (8. Important precautions, 11. Adverse reactions).
When it cannot be used and conditions to disclose
Hypersensitivity to an ingredient, type 1 diabetes and diabetic ketoacidosis are among the contraindications. Disclose pancreatitis, gallstones, gastrointestinal disease, reduced kidney function and planned surgery.
Official prescribing information/product documentation (2. Contraindications (patients who must not receive the medicine), 9. Precautions in patients with specific backgrounds).
Other medicines, supplements and everyday precautions
Avoid combining it yourself with other semaglutide products or GLP-1 medicines. Concurrent insulin or sulfonylureas require attention to hypoglycemia. Also disclose oral medicines and planned anesthesia.
Official prescribing information/product documentation (10. Interactions, 14. Precautions for administration/use).
Pregnancy and breastfeeding precautions
Do not use it during pregnancy or possible pregnancy. If planning pregnancy, discuss when to stop in advance, taking account of how long the medicine remains in the body. Breastfeeding is also assessed individually.
Storage and maintaining product quality
Refrigerate and protect from freezing. Check conditions and permitted duration during use for the specific pen. If discoloration, foreign material or freezing is suspected, do not use it; consult the pharmacy.
Official prescribing information/product documentation (Storage and shelf life, 14. Precautions for administration/use).
Differences from related medicines
Ozempic and Rybelsus are separate products for diabetes treatment in Japan. Even with the same ingredient, their dosage forms, milligram doses and administration intervals are not directly interchangeable.
Official prescribing information/product documentation (4. Indications, 6. Dosage and administration, 18. Pharmacology).
What to discuss with a doctor or pharmacist
Clarify associated health problems, the course of diet and exercise therapy, nutritional status and other treatments, as well as BMI. For diabetes, sleep apnea or liver disease, check the respective specialist diagnosis and medicine indication.
This article provides general information about the medicine. Our clinic does not currently offer prescriptions for this medicine. Points for safer use
- Confirm the route—oral, topical or injection—and the actual product name and strength.
- Distinguish approval in Japan, off-label use and descriptions in overseas documentation.
- Record benefits and side effects together; do not increase the dose yourself or share medicines.
- Tell a healthcare professional about other medicines, supplements, pregnancy, breastfeeding and medical conditions.
