New in 2026

Tadalafil is now sold over the counter — what that changes

Since 31 August 2026, tadalafil — the active ingredient in Cialis — has been on sale in pharmacies across Japan without a prescription, following a limited launch on 31 July. It is the first ED medication in the country to be sold this way.

If you want one or two tablets today and there is a pharmacy near you that stocks it, buying it there is a perfectly sensible thing to do. It is worth knowing what the two routes actually differ on, because they are not the same product on the same terms.

The over-the-counter product is classified as 要指導医薬品 — a category that requires a pharmacist to check suitability in person before the sale. It is limited to men aged 18 and over. Because of that classification it cannot be ordered online, which is the practical difference for most people reading this page.

The other difference is what happens if 10mg is not the right answer for you. A pharmacy can only sell you the one strength. A doctor can change the drug, change the dose, look at what else you take, and tell you when the problem is not one that a tablet solves.

Choosing

Which of the three suits you

Three ingredients, three different habits. None is stronger than the others in any simple sense — they differ in how fast they arrive, how long they stay, and how much a meal gets in the way.

How the three compare
IngredientStarts workingLastsEffect of foodSuits
Sildenafil30–60 min4–5 hoursA fatty meal weakens it mostOccasional use, planned in advance
Tadalafil1–2 hoursUp to 36 hoursLeast affectedNot wanting to plan around a tablet
VardenafilSimilar to sildenafil4–5 hoursLess sensitive to a light mealTaking it after dinner

Sildenafil is where most people start: it costs the least per tablet here, and it has been in use the longest. Moving to tadalafil tends to happen when planning around the timing turns out to be the part that gets in the way. Ordering a small quantity first is possible — nothing is sold as a course, so trying one drug before committing to more is straightforward.

How it works

Five steps, no travel

  1. Message us on LINESay you would like to be seen in English and we will set up the questionnaire and a consultation time with you. Both the booking system and the questionnaire are in Japanese, which is why we do this part with you.
  2. Fill in the questionnaireSent before the appointment. It covers your symptoms, your medical history and anything you already take. This is the part that decides whether it is safe to prescribe, so be accurate about heart and blood pressure medicines.
  3. Speak with the doctorAbout 10 to 15 minutes. The doctor confirms what is suitable and explains the risks before anything is prescribed.
  4. Pay the invoiceSent over LINE after the consultation. Credit card or bank transfer.
  5. Medication is postedDispatched once payment clears, to your home or any address you nominate. Postage is ¥550 per delivery.
Delivery

What arrives, and how

  • The tablets you were prescribed, and nothing announcing what they are. The parcel does not carry the treatment name on the outside.
  • Posted to any address in Japan — home, office, or somewhere else you nominate.
  • ¥550 per delivery, not per item. Ordering two drugs at once is one postage charge.
  • Dispatched after payment clears, not at the moment of the consultation.
  • If you would like the active ingredient and strength written out in English to keep with the medication, ask at the consultation.

What the medication actually looks like

These are the preparations kept in stock, photographed as they are packed. Which one you are sent depends on what the doctor prescribes.

Ordering two of them is still one ¥550 postage charge. The parcel does not carry the treatment name on the outside.

Repeat orders

Ordering again

Once you have been seen, ordering more is short. Message the clinic on LINE, say what you would like and how much, and the doctor reviews it before anything is dispatched — a repeat is still a prescription, not a shop order.

  • The review is brief where nothing has changed. Tell us if anything has — a new medication, a new diagnosis, a change in blood pressure treatment.
  • You can change drug or strength between orders. Many people try sildenafil first and move to tadalafil.
  • One postage charge per delivery, so a larger order costs the same ¥550 to send as a small one.
  • There is no standing subscription. Nothing is charged unless you ask for it.
Taking it

How to take it

  • One tablet, swallowed with water, about an hour before sex. The orally disintegrating tablet dissolves on the tongue and needs no water.
  • Never more than one tablet in 24 hours. Taking two does not double the effect; it multiplies the side effects.
  • It does not work on its own. These medicines make an erection possible in response to arousal — nothing happens without sexual stimulation. This is the most common reason a first tablet is judged a failure.
  • Eat lightly beforehand. A heavy or fatty meal delays and weakens the effect, sildenafil most of all.
  • Keep alcohol light. Alcohol in quantity works against an erection by itself, and adds to the flushing and the drop in blood pressure the tablet can cause.
  • Grapefruit juice raises the blood level of these drugs. Avoid it around the time you take one.

A first tablet not working is not a verdict. Dose, timing and what you ate all move the result. Message us on LINE and the doctor can advise before you order again.

Safety

Side effects, and who must not take this

These are prescription medicines. The doctor reviews your history and everything you already take before anything is prescribed — which is the reason the consultation exists rather than being a formality.

  • Headache is the most commonly reported effect.
  • Facial flushing and a blocked nose are common and pass as the drug wears off.
  • Indigestion or heartburn can occur, more so with sildenafil.
  • Mild visual disturbance — a blue tinge or increased brightness — is reported with sildenafil in particular and is temporary.
  • Back or muscle ache is reported more with tadalafil, usually within a day or two.
  • Sudden loss of vision or hearing is very rare but is a reason to stop and seek medical attention immediately.

Nitrates — the absolute contraindication

ED medication must never be taken with nitrate drugs used for angina and some heart conditions — nitroglycerin, isosorbide dinitrate or mononitrate, in tablet, spray or patch form — or with riociguat. The combination can cause a dangerous fall in blood pressure. This is not a caution to weigh up; it is a hard line.

Prescribing is also not appropriate, or needs particular care, with:

  • Severe cardiovascular disease, unstable angina, or a recent heart attack or stroke
  • Severely low or uncontrolled high blood pressure
  • Severe liver impairment
  • Retinitis pigmentosa
  • Alpha-blockers and some blood pressure medicines — often manageable, but the timing and the dose have to be adjusted
  • Certain antifungals, antibiotics and HIV medicines, which raise the blood level of these drugs

Tell the doctor everything you take, including anything bought without a prescription. If it is not safe to prescribe, the doctor will say so and will not prescribe.

Premature ejaculation

Premature ejaculation can be treated in the same consultation

Premature ejaculation and ED are different problems and are often confused. If the difficulty is the timing rather than the erection, the medication is different: dapoxetine, dispensed here as Poxet at ¥1,500 a tablet.

  • Taken 1 to 3 hours before sex, not daily.
  • Once in 24 hours at most.
  • Avoid alcohol with it — the combination worsens the dizziness and fainting it can cause.
  • The commonly reported effects are nausea, dizziness, headache and diarrhoea.
  • It can be prescribed alongside ED medication when both apply.

About this unapproved medicine

  • Dapoxetine is not approved in Japan. It has no approval as a pharmaceutical under Japanese law.
  • How we obtain it. It is imported personally by the physician, under their own responsibility, and used in self-pay treatment.
  • There is no approved alternative in Japan. No medicine is approved in Japan for premature ejaculation at all, so there is no domestic product to choose instead.
  • Safety information from other countries. Dapoxetine is approved in more than 60 countries, first in Europe in 2009 under the brand name Priligy. It is not approved by the US Food and Drug Administration. The adverse effects reported in those countries are nausea, dizziness, headache and diarrhoea, and fainting has been reported, which is why alcohol is to be avoided with it.
  • Because it is personally imported, it is not covered by Japan’s public relief system for adverse drug reactions (医薬品副作用被害救済制度). If a serious adverse reaction occurred, that scheme would not apply.
  • Treatment on this page is self-pay. Japanese national health insurance does not apply, the price is as listed above, and the side effects and cautions are set out here and gone through again at the consultation.
What you need

No insurance card, no residence card

Treatment here is self-pay, which means national health insurance is not involved either way. That removes most of what usually makes a Japanese clinic difficult to use if you are not a long-term resident.

  • No insurance card. There is nothing to claim against, so none is needed.
  • No residence card and no passport. Your name, a contact number and a delivery address are enough.
  • Any address in Japan — it does not have to be a registered one.
  • Credit card or bank transfer. Invoiced over LINE after the consultation.
  • Nothing is reported to an employer or a family insurance plan.
In person

When coming in is the better option

Online works for a prescription. It does not work for everything, and the doctor will say so rather than prescribe around a problem.

  • Where an online consultation is not enough to prescribe safely — a complicated cardiac history, for instance — you will be asked to attend.
  • If the underlying issue may not be one a tablet solves. Persistent ED can be a sign of something else worth looking at.
  • If you want the medication the same day. At the clinic it is dispensed on the spot; online it is posted.

The clinic is one minute from Shimbashi Station and one stop from Ginza, open until 21:00 on weekdays. Details are on the ED treatment page for in-clinic visits.

Before you start

What you need to know

Important information

  • All treatment on this page is self-pay (自由診療). Japanese national health insurance does not apply.
  • A doctor decides what is appropriate. Where an online consultation is not sufficient to prescribe safely, you will be asked to attend the clinic.
  • Medication is dispatched only after payment has cleared.
  • Dapoxetine (dispensed here as Poxet) is not approved in Japan. It is imported personally by the physician under their own responsibility, and is not covered by Japan’s public relief system for adverse drug reactions. The full disclosure is set out above.
  • ED medication must not be taken with nitrate drugs used for angina and some heart conditions, or with riociguat.
  • Any medicine can cause side effects, and none of these works for everyone. The doctor will decline to prescribe where it is not safe to do so.
  • Prices include consumption tax and were correct at the time of writing. They can change without notice.
  • The over-the-counter figures on this page are the publicly announced retail price and specification of the tadalafil product launched in Japan in 2026. They are given for comparison and are not sold by this clinic.

Coming as an English speaker

You can be treated here without speaking Japanese. There is no English-speaking doctor here — consultations are handled with a translation app and written notes, which works well where the decision is straightforward, as it usually is for a prescription. The booking system and the questionnaire are in Japanese, so message us on LINE first and we will set both up with you.

FAQ

Common questions

How much does online ED treatment cost in total?

The consultation is free. You pay for the tablets plus ¥550 postage. Sildenafil is ¥830 a tablet, tadalafil ¥980 and vardenafil ¥1,040, so four sildenafil tablets delivered comes to ¥3,870 all in. There is no booking fee and no subscription.

Do I have to visit the clinic first?

No. The whole thing is done remotely — message us on LINE, fill in the questionnaire, speak to the doctor for 10 to 15 minutes, pay the invoice, and the medication is posted. If the doctor decides an online consultation is not enough to prescribe safely, you will be asked to come in, and that is the only situation where a visit is needed.

Tadalafil is sold over the counter now. Why use a clinic?

For some people the pharmacy is the sensible choice — if you want one or two tablets today and there is a stockist nearby, buy it there. The differences are that the over-the-counter product is 10mg only, comes four tablets to a box at about ¥1,452 a tablet, and has to be bought in person with a pharmacist going through it with you in Japanese. It cannot be bought online. Here you speak to a doctor, other strengths and other drugs are available, and it is posted to you.

How long does delivery take?

Medication is dispatched once your payment clears. You are invoiced over LINE straight after the consultation and can pay by credit card or bank transfer. Postage is ¥550 to any address in Japan.

Which one should I take?

Sildenafil is the long-standing default: it works in about 30 to 60 minutes and covers 4 to 5 hours, but a heavy or fatty meal beforehand weakens it. Tadalafil comes on more gently and stays for up to 36 hours, which suits people who would rather not plan around a tablet. Vardenafil works in a similar time to sildenafil and is described as less sensitive to a light meal. The doctor goes through this with you.

Do I need to speak Japanese?

No. There is no English-speaking doctor here — the consultation is handled with a translation app and written notes, which is usually enough for a prescription decision. Message us on LINE beforehand and we will set up the questionnaire and the booking with you, since both are in Japanese.

Do I need an insurance card, a residence card or a passport?

No. Treatment here is self-pay, so Japanese national health insurance is not involved and no insurance card is needed. You are not asked for a residence card or a passport either — your name, a contact number and a delivery address are enough.

Can I get this if I do not live in Tokyo?

Yes, that is the point of it. Medication is posted anywhere in Japan, so where the clinic is does not matter for anything that can be treated with a prescription.

What are the side effects?

The commonly reported ones are headache, facial flushing, nasal congestion, indigestion and mild visual disturbance. They are usually mild and pass as the medication wears off. An erection lasting more than four hours is a medical emergency and needs immediate hospital treatment rather than waiting to see.

I take medication for my heart or blood pressure. Is it safe with that?

That is exactly what the consultation is for. ED medication must never be combined with nitrate drugs used for angina, or with riociguat — the combination can drop blood pressure dangerously. Alpha-blockers and several other medicines also interact. Tell the doctor everything you take. If it is not safe to prescribe, the doctor will say so.

Can I order more than a few tablets at once?

Yes. The doctor decides the quantity, and there is no fixed cap the way there is on a pharmacy box. Say at the consultation how many you would like. Nothing is sold as a course, so you are not signing up to a monthly plan.

Can I take the medication out of Japan?

We can write the active ingredient and the strength in English so that a doctor or a pharmacist abroad can read what you were given. We cannot advise on another country’s customs or import rules; please check those yourself before you travel.

Do you treat premature ejaculation online as well?

Yes, with dapoxetine, dispensed here as Poxet at ¥1,500 a tablet. There is no medicine approved in Japan for premature ejaculation, so it is imported by the physician under their own responsibility — please read the section on this page about what that means before you decide.

Last updated . Information on this page was accurate at the time of writing. All treatment here is self-pay; Japanese national health insurance does not apply. Dapoxetine is not approved as a pharmaceutical in Japan. Prescribing follows a consultation with a doctor.
