Transamin is a prescription medicine containing tranexamic acid. Its approved uses in Japan include certain types of bleeding and symptoms associated with inflammation, but not melasma. When treating melasma, we discuss its off-label status and risks. PMDA: Japanese Transamin prescribing information

How to take it and continuing treatment

Follow the instructions provided during your consultation and on the medicine bag for the dose, frequency and timing. Do not increase the dose yourself. Some over-the-counter products contain the same ingredient, so check before combining them.

If your skin changes little or your symptoms change, consult your doctor instead of increasing the medicine. Whether to continue and for how long should also be reviewed during the consultation. See basic administration guidance and evaluating effects and discussing when to stop.

About its effects

Comparative trials have studied oral tranexamic acid for melasma. However, the products, doses and participant results in those studies cannot be directly applied to each person treated at our clinic. A study of effects on melasma

There are different types of pigmentation, so it is important to establish whether it is melasma. The same effect should not be assumed for all dark spots or acne marks. Prescribing for melasma is off-label, and neither the degree nor timing of improvement is guaranteed. See differences in uses and treatment of melasma and melasma versus other pigmentation.

Side effects and precautions

Nausea, diarrhoea, heartburn, loss of appetite, rash, itching and drowsiness may occur. Particular assessment is needed for people with reduced kidney function or receiving dialysis. Convulsions have been reported in dialysis patients.

Tell your doctor if you have blood clots, are at risk of thrombosis, or are confined to bed for a prolonged period after surgery. The medicine may stabilise blood clots, so decisions must take your medical history into account. Prescribing information §§9 and 11

If you develop sudden breathlessness, severe chest pain, or sudden swelling of the lips, tongue or throat while taking it, seek emergency medical care rather than waiting for a routine online appointment. NHS: side effects and when to seek emergency care

Other changes in your health are also covered in symptoms requiring stopping treatment or urgent medical attention. Do not endure symptoms and simply continue; contact the healthcare provider that prescribed the medicine.

Medicines and health conditions to review before prescribing

The medicine must not be administered to people using thrombin. Tell your doctor about all medicines you use, including other medicines affecting haemostasis, hormonal medicines such as contraceptive pills, and over-the-counter products. Kidney disease, drug allergies, possible pregnancy and breastfeeding also need to be reviewed. A prescription may not be appropriate following the assessment. Prescribing information §§2, 9 and 10

Differences from over-the-counter melasma medicines

Over-the-counter products containing tranexamic acid and prescription Transamin are not the same product. Their ingredient combinations and administration differ. Approval of an over-the-counter melasma product in Japan does not establish a melasma indication for prescription Transamin. Check the ingredients before taking them together. Off-label use

Transamin itself is a prescription medicine approved in Japan, but use for melasma is outside its approved indications. A doctor determines whether prescribing is appropriate after reviewing not only the expected effects but also risks such as blood clots and other medicines being used.

Frequently asked questions

Transamin is stored at room temperature. Follow the precautions on the medicine bag and packaging supplied with your prescription. See storage guidance.

Does JPY 2,400 include consultation and delivery?

Will Transamin always be prescribed if I request it?
